CAHAN Enrollment

 

Thank you for your interest in becoming a participant in the California Health Alert Network!

You may enroll in CAHAN using one of the following three ways:

 

  1. Los Angeles County Department of Public Health Enrollment Link

    Click here to request access to CAHAN by filling out your information on this online form:

  2. Request a postage paid enrollment card by sending an email to hanhelp@ph.lacounty.gov. Once you receive the card, fill out your information, and mail it back to the Los Angeles County Department of Public Health. If you have questions or need assistance regarding enrollment, please email hanhelp@ph.lacounty.gov.
     
  3. Contact the CAHAN Team at cahaninfo@cdph.ca.gov or call toll-free at 1-877-376-4767.

 

Participation is generally limited to the medical and healthcare provider community in Los Angeles County and key public health partners.
