Virtual Regional Meetings
January 18 – January 28, 2013
Greetings California Department of Public Health (CDPH) Chronic Disease Prevention Stakeholders:
Due to the overwhelming response from our stakeholders, the regional meetings will be changed from an in-person forum to a virtual meeting forum to accommodate more people. The virtual meetings will be conducted via webinar, and scheduled by region from January 18 through January 28, 2013.
The virtual meetings will provide an opportunity to share CDPH’s proposed California Chronic Disease Prevention Plan goals and strategies, to solicit your input on the strategies and indicators for measuring long-term outcomes, and to gain information of your organization’s activities in chronic disease prevention.
You will have the opportunity to participate in one virtual regional meeting, ideally the one representing your region. See registration information below and sign-up today!

Regional Meeting Goal:
To engage state and local community partners in facilitated discussions to acquire input on for the California Chronic Disease Prevention Plan (State Plan). The State Plan is intended to create a road map with CDPH state and community stakeholders to work collectively to reduce the burden of chronic disease in California. The January 2013 regional stakeholder meetings build on the momentum and feedback received from our initial partner meeting on July 26, 2012.

Coordinated Chronic Disease Prevention Program (CCDPP) Background
In September 2011, CDPH received a three-year grant from Centers for Disease Control and Prevention (CDC) to create a Coordinated Chronic Disease Prevention and Health Promotion Program (CCDPP). The program’s purpose is to build and strengthen CDPH’s capacity and expertise to effectively prevent chronic disease and promote health. CDC would like health departments to focus on the following four key areas: advance environmental strategies for healthful communities; improve health care systems; expand community clinical linkages, and increase regular access to chronic disease and risk factor epidemiologic information.
The CCDPP goal is to create a roadmap with statewide partners to prevent disease and disability, decrease health disparities, improve the quality of health care and empower communities to create healthier environments by participating in statewide multi-sector chronic disease prevention coalitions to guide the development of a multi-year State Plan and support its implementation.
The State Plan is a deliverable to the Centers for Disease Control and Prevention (CDC) Supplemental Funding Opportunity Announcement RFA-DP09-9010301PPHF11, which was awarded on September 1, 2011. The State Plan deliverable is due to CDC on March 30, 2013.
