Conference Registration

 

Early Registration Deadline Extended to May 22, 2009!

Conference registration includes one reception, one full breakfast, two continental breakfasts and all conference materials.

 

You must register for pre-conference sessions in addition to the conference. Pre-conference sessions vary in cost.

 

Cancellations

Cancellations must be received in writing by May 8, 2009, and are subject to a $50 fee. After May 8, substitutions will be accepted but refunds will not be provided. No-shows will be billed the full registration fee.

 

 

CEUs

 

The California State University, Sacramento, College of Continuing Education is an approved provider for the following:

 

Registered Nurses (RN): Provider approved by the California Board of Registered Nursing (BRN), Provider Number 9655. Course meets the qualifications for 14.0 hours of continuing education credit for RN's as required by the CA BRN (Charge $55, partial credit is not available).  

 

Marriage and Family Therapists (MFT), Licensed Clinical Social Workers (LCSW): Provider approved by the California Board of Behavioral Sciences (BBS), Provider Number PCE 4089. Course meets the qualifications for 14.0 hours of continuing education credit for MFT's and/or LCSW's as required by the CA BBS (Charge $55, partial credit is not available).

 

Certified Health Education Specialists (CHES): Provider Number SEP3160. Provider of 15.0 Category I continuing education contact hours (CECH) in health education by the National Commission for Health Education Credentialing, Inc.

 

Registered Dieticians (RD), Dietetic Technicians, Registered (DTR): This program has been approved by the Commission on Dietetic Registration for 17.0 CPEUs.

Accreditation

Long Beach Memorial Medical Center is accredited by the Institute for Medical Quality/California Medical Association (IMQ/CMA) to provide continuing medical education for physicians. Long Beach Memorial Medical Center takes responsibility for the content, quality and scientific integrity of this CME activity.

 

Designation

Long Beach Memorial Medical Center designates this educational activity for a maximum of 25.0 AMA PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate with the extent of their participation in the activity. This credit may also be applied to the CMA Certification in Continuing Medical Education.

 

Nurses - The California Board of Registered Nursing recognizes category 1 courses approved for credit by the American Medical Association toward meeting the continuing education requirements for license renewal.

 

Pharmacists - Coursework which meets the standard of relevance to pharmacy practice and has been approved for continuing education by the Medical Board of California, shall, upon satisfactory completion, be considered approved continuing education for pharmacists.

EXHIBITORS

 

Exhibitor registration is $425 (Non-Profit) and $600 (Commercial) and includes admittance of one person to staff your exhibit, nametag, one 6’ table, one chair, signage and all conference meals and materials. All additional exhibit staff must pay an attendee registration fee of $350 (on or before May 8, 2009) or $400 (after May 8, 2009). A $50 charge will be made for electrical access. Requests must be in advance and noted on the registration form. On-site electrical access requests will not be accommodated.

 

Applications must be received by the NEW DEADLINE of Friday, April 24, 2009 at 3:00 p.m. (PST). Specific information on exhibiting will be distributed with letters of acceptance in late April.

 

 

POSTER PRESENTERS

 

The Poster Sessions at the 5th Biennial Childhood Obesity Conference offers your organization an opportunity to spotlight your program’s creative efforts to address childhood obesity. Poster presentations should display information on the interventions and strategies employed by your project. Presenters may illustrate their projects by displaying photos, diagrams or program poster/banners on available free-standing poster boards (approx. 4’ x 5’) provided. All text should be large enough to read comfortably. During assigned time periods, presenters informally discuss their poster with conference attendees.

 

Applications must be received by the NEW DEADLINE of Thursday, April 30, 2009. You must be registered for the conference. Specific information on displaying posters will be distributed with letters of acceptance in late April.
