For a list of items to submit, please find the situation that applies to you from the list of topics below. Please submit all items through the PPSC Paperwork Submittal link.

If you are a current MSW II student at Sac State, you need to submit the following:

• PPSC Verification of Field Requirement Form
• Copies of MSW II Field Evaluations (Fall & Spring)
• Copy of Field Instructor’s/Supervisor’s valid PPSC or current PPSC document number
• Satisfy the Basic Skills Requirement by providing a copy of a passing score on the CBEST or CSET, or Verification of Basic Skills Requirement 
• Copy of Certificate of Clearance (Important: Your personal profile must be created on the CTC website on or before the day you get your fingerprints taken. You must answer the personal fitness questions and pay for your Certificate of Clearance. Find LiveScan locations.)
• Credential Request Form (provided through submittal link above)
• Unofficial Transcript showing that MSW degree has posted
• Official bachelor’s degree transcripts (except if from Sac State)
• Field administrative/verification fee of $1275 (you are not required to pay this fee as you are paying for your field course through main campus)

If you are a post-master’s student who completed fieldwork during your 2nd year of your MSW program, you need to submit the following:

• PPSC Verification of Field Requirement Form
• PPSC Field Evaluation
• Copies of MSW II field evaluations [fall, spring & winter (if applicable)]
• Copy of Field Instructor’s/Supervisor’s valid PPSC or current PPSC document number
• Satisfy the Basic Skills Requirement by providing a copy of a passing score on the CBEST or CSET, or Verification of Basic Skills Requirement 
• Copy of Certificate of Clearance (Important: Your personal profile must be created on the CTC website on or before the day you get your fingerprints taken. You must answer the personal fitness questions and pay for your Certificate of Clearance. Find LiveScan locations.)
• Credential Request Form (provided through submittal link above)
• Field administrative/verification fee of $1275 (you do not need to submit if you completed your PPSC fieldwork at Sac State)

If you are a post-master’s student who completed fieldwork outside of your MSW program, you need to submit the following:

• PPSC Learning Agreement
• PPSC Verification of Field Requirement Form
• PPSC Field Evaluation
• PPSC Field Instructor’s/Supervisor’s Written Evaluation
• Copy of Field Instructor’s/Supervisor’s valid PPSC or current PPSC document number
• Satisfy the Basic Skills Requirement by providing a copy of a passing score on the CBEST or CSET, or Verification of Basic Skills Requirement
